Hospice Services

Choosing to specialize in hospice care was a deeply personal decision. I’ve always felt a profound connection to helping individuals and families navigate the complexities of serious illness and end-of-life care. For me, hospice is about more than just managing symptoms—it’s about providing comfort, understanding, and support during one of life’s most significant transitions.

Hospice - Sacred Heart of Jesus

You can expect 1 to 2 visits a week. Twenty-four a day availability for symptom management and support. Case management to order standard hospice supplies and equipment. Visits will consist of educating you about incations of end of life, uses, actions, side and adverse effects of medications. I will teach you how to care for your loved one in a tender and loving way to maintain their comfort and dignity.

Transitioning

As they transition closer to their end of days, I will offer to increase the amount the days I visit. If allowed, I will assess the client to ensure she/he is comfortable, clean, dry, and safe. I will check medication and patient product care supplies before departing.

After Care

Post-Mortem care consists of a light wash up and change of clothes if the family wishes. Funeral home of choice will be notified. Medications will be destroyed or delivered to pharmacy. Durable medical equipment company will be notified to pick up the equipement at a time convenient for the family.
